Dear <Name of Employer/Organization>,
I am writing you to make you aware that I hold specific religious beliefs which prevent me from taking a COVID vaccine or COVID test. These and certain other medical procedures/applications violate my deeply held convictions in the Christian faith.
The 1st Amendment of the US Constitution protects the free exercise of my faith in all arenas of life, both private and public, and accordingly I am exercising my federally protected rights under 42 USC § 2000e–2(a)(et seq) which read as follows:
Unlawful employment practices
(a) Employer practices
It shall be an unlawful employment practice for an employer—
(1) to fail or refuse to hire or to discharge any individual, or otherwise to discriminate against any individual with respect to his compensation, terms, conditions, or privileges of employment, because of such individual’s race, color, religion, sex, or national origin; or
(2) to limit, segregate, or classify his employees or applicants for employment in any way which would deprive or tend to deprive any individual of employment opportunities or otherwise adversely affect his status as an employee, because of such individual’s race, color, religion, sex, or national origin.
Per United States Federal Labor and Employment Law, my religious beliefs cannot be used as grounds for termination, retaliation or discrimination. While I am under no obligation to be specific with you of these beliefs, to enumerate them, or be asked or required to defend them, I am available to discuss my faith so as not to be misunderstood, or my resolve misconstrued. I am also voluntarily providing a statement of faith on this matter at the end of this letter.
Please place this letter in my appropriate HR file as a substitute for any such proof or certificate of vaccine or proof of testing that you may need.
Thank you.

Statement of Faith
Re: COVID Vaccine Mandates, COVID Test Mandates, COVID Mask Mandates
Jesus clearly teaches that our primary obligation is to love God first and foremost, and to obey Him first. Disobeying God’s Word and instructions in order to follow the laws, policies, edicts, emotions or whims of people or pop-culture is idolatry (sin).
• Matthew 22:35-39 - Then one of them, which was a lawyer, asked him a question, tempting him, and saying, Master, which is the great commandment in the law? Jesus said unto him, Thou shalt love the Lord thy God with all thy heart, and with all thy soul, and with all thy mind. This is the first and great commandment. And the second is like unto it, Thou shalt love thy neighbour as thyself.
• Acts 4:18-20 - And they called them, and commanded them not to speak at all nor teach in the name of Jesus. But Peter and John answered and said unto them, Whether it be right in the sight of God to hearken unto you more than unto God, judge ye. For we cannot but speak the things which we have seen and heard.
• Acts 5:29 - Then Peter and the other apostles answered and said, We ought to obey God rather than men.
• Romans 12:2 - And be not conformed to this world: but be ye transformed by the renewing of your mind, that ye may prove what is that good, and acceptable, and perfect, will of God.
• John 14:15 - If ye love me, keep my commandments.
Christianity governs the physical realm; it is not just an invisible belief system. As a follower of Jesus, my Creator has sole ownership over my body. It does not belong to the government, an employer, the medical community, or society to do with it as they wish. God alone has the authority to issue mandates for my body.
• 1 Corinthians 6:19-20 - What? know ye not that your body is the temple of the Holy Ghost which is in you, which ye have of God, and ye are not your own? For ye are bought with a price: therefore glorify God in your body, and in your spirit, which are God's.
• 1 Corinthians 3:17 - If any man defile the temple of God, him shall God destroy; for the temple of God is holy, which temple ye are.
The Lord has sole rights to my body, and He has made me its custodian. As such, He expects me to honor Him with my body. I do not engage in sexual sin, because that is sin committed with my body. I do not inject by body with drugs, medical experiments, gene therapy, poisons or the cells of aborted babies, no matter who around me may be germophobic or fearful. I do not take medications for the same reasons. In addition, I do not subject my body to possible injury or infections through medical trials, tests, procedures, or mandates that limit its ability to naturally function.
• Romans 12:1-2 - I beseech you therefore, brethren, by the mercies of God, that ye present your bodies a living sacrifice, holy, acceptable unto God, which is your reasonable service. And be not conformed to this world: but be ye transformed by the renewing of your mind, that ye may prove what is that good, and acceptable, and perfect, will of God.
Jesus speaks of a time when governments of the world will (erroneously) believe that they are God. As such, they will overreach their boundaries, and use coercion to institute draconian measures on society to lock it down and control it. Its tool of choice is called the “mark” in the Bible, which in the original language of the Scripture is referred to as a “jab”. The mark or jab is injected into the body, and it is used to determine who can conduct business, who can engage in commerce, who can buy or sell or be employed, and who has freedoms within society. The Bible clearly teaches that this is demonic, and Jesus’ followers are not to participate in this system of control, or any of the precursor events that set the stage for it.
• Revelation 14:9-12 - And the third angel followed them, saying with a loud voice, If any man worship the beast and his image, and receive his mark…The same shall drink of the wine of the wrath of God, which is poured out without mixture into the cup of his indignation; and he shall be tormented with fire and brimstone in the presence of the holy angels, and in the presence of the Lamb: And the smoke of their torment ascendeth up for ever and ever: and they have no rest day nor night, who worship the beast and his image, and whosoever receiveth the mark of his name. Here is the patience of the saints: here are they that keep the commandments of God, and the faith of Jesus.
• Revelation 16:2 - And the first went, and poured out his vial upon the earth; and there fell a noisome and grievous sore upon the men which had the mark of the beast, and upon them which worshipped his image.
• Revelation 19:20 - And the beast was taken, and with him the false prophet that wrought miracles before him, with which he deceived them that had received the mark of the beast, and them that worshipped his image. These both were cast alive into a lake of fire burning with brimstone.
• Romans 12:2 - And be not conformed to this world: but be ye transformed by the renewing of your mind, that ye may prove what is that good, and acceptable, and perfect, will of God.
• 1 John 2:15 - Love not the world, neither the things that are in the world. If any man love the world, the love of the Father is not in him.
• 1 John 5:4 - For whatsoever is born of God overcometh the world: and this is the victory that overcometh the world, even our faith.
• 2 Corinthians 3:17 - Now the Lord is that Spirit: and where the Spirit of the Lord is, there is liberty.
In matters of sickness, disease, pestilence and pandemic, my faith and trust rests entirely in the Lord. The medical community can help with injuries, but its abilities with chronic disease and viral infection are experimentation at best, and it can cause more harm than good. My body is not a guinea pig for anyone, and I trust God alone for my health and healing.
• Exodus 23: 25 - And ye shall serve the Lord your God, and he shall bless thy bread, and thy water; and I will take sickness away from the midst of thee.
• Exodus 15:26 - And said, If thou wilt diligently hearken to the voice of the Lord thy God, and wilt do that which is right in his sight, and wilt give ear to his commandments, and keep all his statutes, I will put none of these diseases upon thee, which I have brought upon the Egyptians: for I am the Lord that healeth thee.
• Isaiah 53:5 - But he was wounded for our transgressions, he was bruised for our iniquities: the chastisement of our peace was upon him; and with his stripes we are healed.
• 1 Peter 2:24 - Who his own self bare our sins in his own body on the tree, that we, being dead to sins, should live unto righteousness: by whose stripes ye were healed.
• Psalms 103:2-3 - Bless the Lord, O my soul, and forget not all his benefits: Who forgiveth all thine iniquities; who healeth all thy diseases;
• Psalms 91:3-6 - Surely he shall deliver thee from the snare of the fowler, and from the noisome pestilence. He shall cover thee with his feathers, and under his wings shalt thou trust: his truth shall be thy shield and buckler. Thou shalt not be afraid for the terror by night; nor for the arrow that flieth by day; Nor for the pestilence that walketh in darkness; nor for the destruction that wasteth at noonday.
• Psalms 91:9-11 - Because thou hast made the Lord, which is my refuge, even the most High, thy habitation; There shall no evil befall thee, neither shall any plague (disease) come nigh thy dwelling.
Because I have put my faith and trust in Jesus Christ, I do not carry disease to others. I am a super-spreader of the solution, not the problem. Scripture instructs me to lay hands on the sick and pray for their healing, not quarantine myself or isolate myself from them.
• Matthew 10:8 - Heal the sick, cleanse the lepers, raise the dead, cast out devils: freely ye have received, freely give.
• Mark 16:17-18 - And these signs shall follow them that believe…they shall lay hands on the sick, and they shall recover.
• James 5:14-15 - Is any sick among you? let him call for the elders of the church; and let them pray over him, anointing him with oil in the name of the Lord: And the prayer of faith shall save the sick, and the Lord shall raise him up; and if he have committed sins, they shall be forgiven him.
In short, this is a matter of faith and what/who I sincerely believe. I believe that Jesus is the only solution for the world’s problems today, and if you haven’t entrusted your life to Him completely, you may want to consider doing so. Don’t hesitate to contact me if you have any questions.
